Below are forms and information regarding Adventure Camp

		Attendee Release and Parent Authorization Form Form must be completed and returned by January 26, 2009 Attendee Release and Parent Authorization Form Spanish
		Letter of Intent Form must be completed and returned by January 26, 2009
		Medical Release Form Form must be completed and returned by January 26, 2009
		Medication Form Page 1 Page 2 Form and any medication must be turned into the School Nurse no later than May 18, 2009
		Cost of Camp and Payment Dates Expense of Camp
		Bring Along List List of items to bring and not to bring...........that is the question.
